- DMV printout of driving record
- Vehicle inspection
- Physical & TB Test for all adults in the home
- Caretaker Plan & Regulations
- 20 hours of Pre-Approval training
- CPR/First Aid & Water Safety Certificate

- DCFS-OHC Clearance for all adults
- Live Scan for all adults in the home
- LIS & LAARS for applicants only
